Medicare GLP-1 Bridge Program: Who Qualifies for $50 Wegovy or Zepbound?

Medicare now offers eligible Part D beneficiaries access to certain GLP-1 weight-loss medications, including Wegovy and Zepbound, for a $50 monthly copay through the Medicare GLP-1 Bridge Program. Eligibility depends on your starting BMI, medical conditions, Medicare drug coverage, and prior authorization. This guide explains who qualifies, which medications are covered, and how to apply.


Step 1: Do you have qualifying Medicare Part D drug coverage?

Yes: Continue to Step 2.

No: You generally do not qualify for the Medicare GLP-1 Bridge.

Yes: Continue to Step 3.

No: The Medicare GLP-1 Bridge may not apply. GLP-1 medications prescribed for a condition already coverable under Medicare Part D (diabetes, sleep apnea, etc) generally must be processed through the patient’s regular Part D plan.

BMI of 35 or higher


You may meet the BMI requirement without an additional qualifying medical condition.


BMI of 30 to 34.9


You may qualify if you also have at least one of the following:

  • Heart failure with preserved ejection fraction, or HFpEF

  • Uncontrolled high blood pressure despite treatment with two blood-pressure medications

  • Chronic kidney disease stage 3a or higher


BMI of 27 to 29.9


You may qualify if you also have at least one of the following:

  • Prediabetes

  • A previous heart attack

  • A previous stroke

  • Symptomatic peripheral artery disease


BMI below 27


You do not meet the current Medicare GLP-1 Bridge BMI criteria.



What is the Medicare GLP-1 Bridge Program?


The Medicare GLP-1 Bridge is a temporary federal program that gives eligible Medicare Part D beneficiaries access to certain GLP-1 weight-loss medications for $50 per monthly supply. The program began July 1, 2026, and is scheduled to continue through December 31, 2027.



Does Medicare now cover GLP-1 medications for weight loss?


Yes. Beginning July 1, 2026, eligible Medicare Part D beneficiaries can receive certain GLP-1 medications for weight loss through the Medicare GLP-1 Bridge. Coverage is not automatic: patients must meet the program’s clinical criteria and receive prior authorization.



Who qualifies for the Medicare GLP-1 Bridge?


Eligible adults must have qualifying Medicare Part D coverage, use the medication for weight loss or weight maintenance, and meet one of the program’s BMI-based clinical pathways. A medical provider must prescribe an eligible medication and attest that the patient meets the requirements through prior authorization.

What BMI is required for Medicare GLP-1 coverage?


The Medicare GLP-1 Bridge requires a starting BMI of 35 or higher, a BMI of 30 or higher with certain health conditions, or a BMI of 27 or higher with certain cardiovascular or metabolic conditions. CMS uses the patient’s BMI when GLP-1 treatment was first started—not necessarily their current BMI.



Which medical conditions can help someone qualify?


With a BMI of 30 or higher, qualifying conditions include heart failure with preserved ejection fraction, qualifying uncontrolled hypertension, or chronic kidney disease stage 3a or higher. With a BMI of 27 or higher, qualifying conditions include prediabetes, a previous heart attack, previous stroke, or symptomatic peripheral artery disease.



Which GLP-1 medications does the Medicare Bridge cover?


As of July 2026, the Medicare GLP-1 Bridge covers Foundayo tablets, Wegovy injections and tablets, and the Zepbound KwikPen when prescribed for weight loss or weight maintenance. Covered medications and formulations may change during the program.



Does Medicare cover Zepbound for weight loss?


Yes. Eligible Medicare Part D beneficiaries may receive the Zepbound KwikPen for weight loss through the Medicare GLP-1 Bridge for a $50 monthly copay. The program does not cover Zepbound single-dose pens or vials, and patients must purchase KwikPen needles separately.


Does Medicare cover Wegovy for weight loss?


Yes. The Medicare GLP-1 Bridge covers both injectable Wegovy and Wegovy tablets for eligible Medicare Part D beneficiaries who meet the weight-management criteria. A provider must prescribe Wegovy and submit the required prior authorization before coverage is approved.



How much do GLP-1 medications cost through the Bridge?


Eligible patients pay $50 for each monthly supply of a covered GLP-1 medication through the Medicare GLP-1 Bridge. The copay does not count toward the Part D deductible or annual out-of-pocket spending, and it does not include medical consultations, monitoring, pen needles, or other clinical expenses.



How do I apply for the Medicare GLP-1 Bridge?


Patients do not apply on their own. A medical provider must determine that treatment is appropriate, prescribe an eligible GLP-1 medication, and submit a Medicare GLP-1 Bridge prior authorization. A denial from the patient’s regular Part D plan is not required before using the Bridge.



How does the prior authorization process work?


The pharmacy sends the prescription claim to the Medicare GLP-1 Bridge, which triggers a prior authorization request to the prescriber, usually within 24 to 72 hours. The provider submits the patient’s eligibility information electronically or by fax, and a decision is generally issued within 72 hours after submission.



Does my doctor have to accept Medicare?


No. A provider does not have to be enrolled in or accept Medicare to prescribe a covered medication or submit a Medicare GLP-1 Bridge prior authorization. However, the provider cannot be listed on Medicare’s Preclusion List, and the Bridge does not necessarily cover the provider’s consultation or follow-up fees.



Can I qualify if I am already taking a GLP-1?


Possibly. Previous GLP-1 treatment does not automatically disqualify you, and CMS evaluates eligibility using your BMI when treatment was first started. However, patients already receiving GLP-1 coverage through Medicare Part D generally must continue through their Part D plan rather than switch to the Bridge.



Who does not qualify for the program?


Patients may not qualify if they lack eligible Part D drug coverage, do not meet the BMI and medical criteria, or already receive a GLP-1 through Part D. Patients using GLP-1 therapy for type 2 diabetes, moderate-to-severe sleep apnea, or qualifying MASH generally seek coverage through their regular Part D plan instead.
